Director of Population Health and Quality Analytics to join our team.
Position Details
The
Director of Population Health and Quality Analytics leads efforts to improve population health outcomes, enhance the consumer and provider experience, and demonstrate value and ROI. Ultimately, the Director of Population Health and Quality Analytics, using analytic methodologies, ensures the provision of the highest quality of health and human services and proves the effectiveness of exceptional care, through demonstrable data. The
Director of Population Health and Quality Analytics will:
- Conduct population health analysis by extracting and integrating internal and external health data.
- Ensure relevant health data is captured, analyzed, and interpreted at individual and population levels. Analyze data to determine strategic priorities and interventions needed to enable integrated systems of care, close existing care gaps and improve quality outcomes for individuals and communities.
- Develop methods, processes, or checks to assess and improve data quality across the data life cycle from reporting, entry, access, extraction, and use.
- Deploy risk stratification methodologies to identify at-risk consumers and populations and recommend and support the implementation of targeted strategies.
- Proactively work with stakeholders to prioritize data and analytic needs, partnering with IT/data analysts to develop actionable reporting through models and visualizations to support quality initiatives and care delivery.
- Provide learning opportunities and consultative services to operational leadership and support departments (IT, finance, etc.) on population health and value-based initiatives, data management, risks, and performance.
- Maintain knowledge of value-based purchasing outcomes including HEDIS, NCQA and CCBHC specifications to optimize organization’s readiness and performance.
- Promote a culture of continuous data-driven quality improvement. Assist in the development of quality goals/metrics and support the ongoing monitoring of performance and improvement strategies.
- Support in the development of tools and strategies, as well as monitor and analyze performance data, to help programs exceed targeted benchmarks and maximize incentive payments.
- Direct, mentor, and coach staff and engage resources to develop a high-performing data organization.
- Assist with special projects and/or presentations.
