The ePEP Case Manager-QMHP will function as an intensive case manager and health coach to patients with the first episode of psychosis on the CSC Team Services may include, but are not limited to, providing health coaching, skills training, treatment, assisting with communication to the providers & CSC Team, monitoring symptoms & side effects, working closely with CSC Family Peer Support Specialists on a recovery plan, co-facilitating psychoeducational groups, advocating for the patient, crisis intervention, providing continuity of care, monitoring medication use, and participation in treatment. The overall goal of this position is to assist patients on the CSC team to reach their maximum potential.
Essential Duties And Responsibilities
- Facilitate psychoeducational and recovery groups using a specific curriculum.
- Provide overall service coordination, referral, linkage, and advocacy to patients with varying needs. Perform duties in concert with other members of the CSC Team.
- Participate in CSC team meetings.
- Follow up with assigned patients who missed a scheduled appointment. Document attempts to contact the consumer.
- Follow up with assigned patients within 24 hours on assigned patients who access emergency services. Report status and disposition to the treatment team.
- Provide crisis intervention. Make home visits, initiate mental illness warrants as appropriate, and work with police and other public servants as needed to address crises. Includes on-call services.
- Assist with inpatient admission when applicable. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning.
- Perform other duties as assigned.

Qualifications
EDUCATION AND EXPERIENCE:
- Master's degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
Computer Skills
- Use the computer, printer, and software programs necessary for the position (e.g., Word, Excel, Outlook, PowerPoint)
- Ability to utilize the Internet for resources.
