The PASRR Evaluation/Diversion Coordinator Team Lead is responsible for overseeing and coordinating the PASRR (Preadmission Screening and Resident Review) evaluation and diversion process to ensure individuals with intellectual and developmental disabilities (IDD) receive appropriate services and support. This role involves providing leadership, guidance, and oversight to a team of evaluators, ensuring compliance with state and federal PASRR regulations, and facilitating diversion efforts to prevent unnecessary institutionalization.
HOURS OF RESPONSIBILITY: Monday – Friday 8 am – 5 pm, may include evening and weekend hours depending on task.
Essential Duties And Responsibilities

- Provide supervision, training, and support to Evaluation Coordinators.
- Monitor Evaluation Coordinators, including job evaluations, compliance with policies, and implementation of the disciplinary process when necessary.
- Ensure the timely completion and submission of required documentation, including but not limited to Service Documentation and PASRR Evaluations in accordance with Texas Administrative Code (TAC), Health and Human Services (HHS), and PASRR Billing requirements.
- Monitor staff benchmarks and appropriateness of assigned PE alerts.
- Provide coverage for evaluator responsibilities during staff vacancies or as needed.
- Identify available community living options, services, and supports to assist individuals to successfully live in the community.
- Provide information and assistance to LIDDA staff who are facilitating diversion for people at risk of admission to a nursing facility (NF) and for people transitioning to the community from an NF.
- Coordinate educational activities for LIDDA staff about available community services and strategies to avoid NF admission.
- Coordinate educational activities for referring entities about available community resources, services, and strategies to avoid NF admission.
- Within 45 to 75 calendar days after a person is admitted into an NF, review the person’s admission to ensure that community living options, services, and support that could provide an alternative to NF services have been explored and, if not, refer the person to their rehabilitation coordinator.
- Identify, arrange, and coordinate access to community services as a diversion to NF admission for a person who has chosen community living during the PE process.
- Request a targeted NF Home and Community-based Services (HCS) diversion slot, targeted NF HCS Diversion Slot, or transition slot.
- Performs other duties as assigned.

Qualifications
EDUCATION AND EXPERIENCE:
- Required: Bachelor’s or advanced degree from an accredited college or university required with a major in a social, behavioral, or human service field, including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and criminal justice .
- Preferred: Master's degree in social, behavioral, or human services with a major in a qualified field such as psychology, counseling, or social work
- Supervisory/Management Experience: Requires at least two years of supervisory or management experience.
- Four years' experience in coordinating or providing services to individuals with IDD, including those with complex medical needs, in the community.

Certifications, Licenses, Registrations
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain a Texas Driver License within three (3) months of employment.
- Liability insurance is required if an employee will operate a personal vehicle on Center property or for Center business. Must be insurable by Center’s liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
