Care Coordinator- Assertive Community Treatment Team (ACT)
Role Responsibilities
The ACT Team Care Coordinator works in a fast-paced environment and provides direct services to adult individuals diagnosed with chronic, serious mental illness. Responsibilities include: assessing, planning, coordinating, monitoring, and linking patients to community resources and service providers. The Care Coordinator is responsible for assessing the clients using the standardized assessments, building recovery plans, utilizing evidenced based practices and coordinating within the team. This is a community-based position that provides 80 billable hours, which 80% of these hours are conducted within the community.
Routine hours are Monday through Friday, 8:00 am to 5:00pm with flexibility to meet program needs outside of routine hours. The position reports directly to the ACT Team Leader or Clinical Support Manager and will perform other duties as assigned. Must have reliable vehicle for transportation.
Responsible and accountable for adherence to and promotion of the compliance program.
Salary
- 90% $21.92 hour to start;
- 95% $23.14 hour at 6 mos. with satisfactory performance
- 100% $24.36 hour at 1 yearwith satisfactory performance
Required Education and/or Certifications, Experience, Skills
- Bachelor’s or advanced degree from an accredited college or university
- Texas driver’s license in good standing as defined by Texana Center’s procedure
- 1 year of experience working with adults with serious mental illness
Preferred Education and/or Certifications, Experience, Skills
- Prior experience in a community mental health center
- Spanish speaking
Job Related Physical Requirements
- Must be able to perform SAMA and CPR.
- Must be able to perform moderate lifting and carrying (15-44#), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting.
- Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
- Qualified individual must be able to perform these functions with or without reasonable accommodations.
