Position Overview
As a
Community-Based Substance Use Disorder Case Manager, you will play a key role in supporting individuals in recovery. Your responsibilities will include:
- Case Management: Provide hands-on support and guidance to individuals in recovery, assisting with accessing resources such as housing, medical care, recovery support (e.g., 12-step meetings), and food banks.
- Collaboration: Communicate effectively with Supervisors and Counselors to ensure all client needs are met and that changes in services are communicated in a timely manner.
- Support in Transition: Help clients with referrals to higher levels of care and ensure necessary documentation, such as ROIs, is handled smoothly.
- Motivation & Education: Encourage clients to engage in therapy services and provide education to those who may be resistant to treatment.
