We are hiring a
Registered Nurse (
RN) to provide healthcare coordination and direct intervention, as needed, for designated individuals.
RN will also provide training and clinical oversight for unlicensed staff and LPNs to meet the healthcare needs of individuals served.

Job Responsibilities
- Function as liaison and advocate for individuals’ healthcare with all members of individuals’ medical and interdisciplinary teams. Advocate for their wellbeing and inclusion.
- Train unlicensed staff on medication supervision of self-administration, delegated tasks, treatment protocols, and monitoring.
- Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.
- Oversee pharmacy and medication needs; this may include but is not limited to reviews, obtaining medications, prescriptions, and prior authorizations.
- Order medical supplies from established formularies as applicable for your program.
- Maintain health-related records, complete documentation and assessments per company policy, federal, and state regulations, as applicable.
- Obtain and maintain HRST rater certification.
- Support individuals in obtaining health services appropriate for their preventative, acute, and/or chronic illness needs.
- Oversee that the health status and physical conditions of individuals are assessed (hands-on), monitored, reported, documented, and responded to in a timely and appropriate manner.
- Respond to emergency medical situations in a timely manner, as appropriate.
- Participate in and attend assigned medical individuals’ appointments (as directed), individual specific, nurse, departmental, and other meetings as required.
- Develop and update individual specific plans of care and health care plans to ensure appropriate medical care and supervision for assigned individuals.
- Manage transitions of care to and from hospital, nursing home, rehab facility and/or another agency.
- Provide oversight with physician orders and appointments.
- Advise on potential new admissions in regard to healthcare needs and appropriateness for the program.
- Provide nursing on-call per program requirements.
- Mandatory reporter of any suspected abuse, neglect, or exploitation to the supervisor or Director

Qualifications
- RN with current Georgia license.
- Experience in IDD/Mental Health nursing is preferred.
- Experience working with individuals with intellectual, developmental disabilities and/or mental illness.
- Valid Driver’s License.
